To qualify for mortgage insurance from the FHA or HUD you must complete a loan application.

The application asks for information such as:
  • The borrower's name, phone number, address and other personal information
  • The property addresss
  • The amount of the mortgage loan
  • The type of home to be purchased (pre-existing, new construction, partially-completed construction, manufactured, condominium, duplex, etc.) and the type of mortgage loan (new or refinance)
  • The name, address, and phone number of the current lender (if applicable)
  • The amount of the monthly mortgage payment amount
  • The purchase price of the home
  • The interest rate of the mortgage loan
  • A statement from both the borrower and the lender certifying that all of the given information is correct
